First Visit
Your initial appointment will consist of a consultation to explain your diagnosis and treatment options. Occasionally, surgery can be performed at the initial appointment, however, a complex medical history or treatment plan will require an evaluation appointment and a second appointment to provide the treatment.
Office Hours
Prior to your first visit please fill out a Patient Registration here with secure submission to our office. Please call if you have any questions about the registration process.
Please assist us by bringing the following to your initial appointment:

Your surgical referral slip and any x-rays if applicable. (Email of digital xrays is preferred to [email protected])
- A list of your current medications.
- Your driver’s license or other photo
- If you have medical and/or dental insurance, bring your insurance card(s).
- Payment in the form of cash, check or credit card.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and physical examination is mandatory for patients undergoing IV (general) anesthesia for their surgery. On the day of surgery, please have nothing to eat or drink for at least eight hours prior to your surgery. Please arrange for an adult to drive you home if you are undergoing general anesthesia as you will be unable to drive that day. Your driver will need to stay at our facility during your procedure.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office at [email protected] If xrays are unable to be emailed, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
